    The Check@Home consortium is established by the Dutch Heart Foundation, the Dutch Kidney Foundation, and the Dutch Diabetes Research Foundation and is supported by the Dutch CardioVascular Alliance. Together with researchers, medical experts and industrial partners they aim to develop a nationwide approach for the early detection of cardiovascular disease, chronic kidney disease, and type 2 diabetes.

  • WHAT DO WE AIM FOR

    We aim to develop a roadmap for a national screening program for the early detection and treatment of cardiovascular disease, chronic kidney disease and type 2 diabetes and their underlying common risk factors. Check@Home aims to build an infrastructure that contributes to the sustainability and affordability of healthcare, that empowers citizens by translocating care to their own living environment, and that improves the cost-effectiveness of screening by selecting only those patients for diagnostic work-up and treatment that have early signs of disease.

  • WHERE IS CHECK@HOME

    In four regions across the Netherlands, Breda, Utrecht, Arnhem and Eindhoven, a total of 160.000 individuals between 50 to 75 years of age will be invited to participate in the screening program. Check@Home plans to start with the screening in the first region Breda during the first quarter of 2025, with the remaining regions to follow thereafter.

The  challenge

Cardiovascular disease, chronic kidney disease and type 2 diabetes currently affect 1.55 million, 1.8 million, and 1.2 million people in the Netherlands. The future is alarming as the number of people suffering from these chronic diseases is expected to further increase in the coming years. When these diseases are not detected and treated at an early stage, they can lead to major complications such as kidney failure, stroke, myocardial infarction and heart failure. A lot of people are unaware of having these chronic diseases, as it is often present without overt symptoms. Check@Home aims to overcome this problematic situation by developing a screening program for the early detection and treatment of these chronic diseases and their underlying common risk factors.
