The Check@Home consortium was established in 2020 by the Dutch Heart Foundation, the Dutch Kidney Foundation, and Dutch Diabetes Research Foundation and is supported by the Dutch CardioVascular Alliance. Their aim is to develop a nationwide approach for the early detection of cardiovascular disease, chronic kidney disease, and type 2 diabetes. In addition to these founders, various parties are collaborating within the consortium, including industrial partners (Roche Diagnostics, AstraZeneca, Siemens Healthineers, Happitech, Topicus), medical experts (general practitioners, cardiologists, nephrologists, internists) and specialists in social sciences, health economics, and communication from several Dutch university medical centers, universities, and regional hospitals. The research leaders are cardiologist Prof. Dr. Folkert Asselbergs from Amsterdam UMC and internist-nephrologist Prof. Dr. Ron Gansevoort from UMC Groningen.

OurGOALS
Check@Home aims to develop a roadmap for a national screening program focused on the early detection of cardiovascular disease, chronic kidney disease and type 2 diabetes and their common risk factors. Early detection can lead to more effective treatments, reducing the risk of complications. By lowering the morbidity and mortality associated with these diseases, the burden on both the society at large and individuals in specific can be significantly reduced. Through screening for cardiovascular disease, chronic kidney disease and type 2 diabetes, Check@Home expects to achieve a significant positive impact on the overall health of the society.
To build an infrastructure that contributes to the sustainability and affordability of healthcare
To build a program that is accessible to and empowers individuals across all socio-economic status groups within their own living environment

OURWORK PACKAGES
Work package 0: Management
WP0 includes the overall management of the Check@Home consortium, covering general, scientific, communication, financial, and administrative aspects. Its role is to support the consortium in maintaining coherence, tracking progress, ensuring project unity, and aligning all involved teams and parties.
Work package 1: Home-based screening
WP1 determines which home-based screening approaches will be used in the screening, that are likely to be (cost-)effective in detecting the diseases at an early stage and will assess the added value of each individual home-based screening method, as well as the overall effectiveness of the home-based approach.
Work package 2: Diagnostics
WP2 designs the diagnostic phase of the program where participants with abnormal home-based screening results will be screened in a regional screening facility to determine if they have any risk factors or disease.
Work package 3: Care pathway
WP3 designs the care pathway of the program following the diagnostic screening phase, including the referral route for participants to their general practitioners to receive appropriate care, but also setting up regional treatment facilities in which participants can be treated as part of the screening.
Work package 4: Data infrastructure & data management
WP4 sets up the supportive digital infrastructure required to collect and analyze all outcomes relevant to the project, including the development and maintenance of the app used in the home-based screening phase and the data platform in which all data will be stored and managed.
Work package 5: Impact on health & economic outcomes
WP5 performs the cost-effectiveness assessment of the screening, by providing a comprehensive assessment of screening methods, their health impacts, and cost benefits to support nationwide implementation using dynamic modeling and stakeholder feedback to optimize and validate screening strategies.
Work package 6: Social and ethical aspects of screening
WP6 contributes to evidence-based strategies to increase reach and well-informed choice of citizens to participate in the screening, as well as engagement of general practitioners in the screening process, by qualitative and quantitative assessment of barriers and facilitators for participation in the screening.
Work package 7: Implementation and public affairs
WP7 focuses on identifying the route towards implementation of the Check@Home screening program for the early detection of cardiovascular disease, chronic kidney disease and type 2 diabetes to a national level, by involving important stakeholders and by creating political impact.
Work package 8: Communication and public relations
WP8 is responsible for the project results dissemination and communication, by publications in scientific and laymen journals, presentations at congresses, meetings with relevant stakeholders, and public outreach through involved patient organizations, citizens initiatives, and involved health funds.
